Home and Community Base Services

 

Home and Community based services provides services and support beyond those covered by MA(Medicaid/medical assistance) program that enable a person to remain in a community/home setting rather than being admitted to a nursing facility.

 

A Sheltering Embrace provides your friend or relative with training to become your personal care assistant. If you do not have an adult child, niece, nephew, close friend or relative to provide assistance to you, A Sheltering Embrace will provide you with trained and knowledgeable staff.

 

A Sheltering Embrace makes it possible for you to stay in the community by providing training to a friend or relative. They can then care for you in your home. If necessary, A Sheltering Embrace can also provide trained staff to take on some of these duties.

 

HCBS refers to services offered in the community instead of in a facility. The needs of the individual determine this, and your service coordinator can develop a person-centered service plan to resolve this.

 

Home and community-based services are varied. Some of the services that can be provided include:

 

  • Adult Daily Living Services / Adult Day Care

  • Attendant Care (Personal Care)

  • Community Transition Services (Moving Assistance)

  • Companion Services (Escort)

  • Coordination Services

  • Counseling

  • Environmental Accessibility Modification (Home and/or Vehicle

  • Financial Management Services

  • Home Health Services

  • Home Medical Equipment and Supplies

  • Meal Delivery (Hot or Prepared)

  • Non-Medical Transportation

  • Personal Emergency Response (PER) System

  • Respite Care Services (Temporary Caregiver Relief)

  • Telecare

Caregiver/Personal Care Assistant

​

The Caregiver/Personal Care Assistant is the heart of what we do. These paraprofessionals assist with the client’s activities of daily living and instrumental activities of daily living which can be slightly more complex. All of our aides are vetted and trained.

 

Caregivers/ Personal Care Assistants showcase their warmth and compassion in delivering the highest level of care to those who are most vulnerable and need it most. Seeking the best candidates is the cornerstone of low turn-over and maximizing the chances of ensuring the most compassionate and consistent care.

 

Services include:

 

  • Companionship and conversation

  • Assisting with showers & baths

  • Assisting with dressing

  • Hair, skin, foot, nail and oral care

  • Medication reminders

  • Meal preparation and ensuring proper nutrition

  • Assist with feeding

  • Assist with ambulation (wheelchair, walker, cane, etc.)

  • Clean dentures

  • Laundry, shopping and similar errands

  • Light housekeeping and similar household tasks

  • Community engagement
